Client Intake
Please complete this form as fully as possible. If we are able to offer you a spot in care, we will reach out to you within 10 business days.
First appointments are booked for 9-10 weeks of pregnancy. If you desire care in advance, please visit your family doctor or walk-in clinic.
You will receive a confirmation email that your form has been received.
If we are able to offer you a spot in care, we will reach out to you within 10 business days. If you do not hear from us, you will be placed on our wait list. Spots do become available as late as 20 weeks of pregnancy.
Due to very high demand, we are not able to reply to every applicant. Please do not call or email our clinic to inquire about the status of your application. Responding to these calls takes valuable time away from providing care.
